Summer Camp Forms

All forms are due no later than Noon the Thursday prior to your child’s start date.

According to Connecticut State law, campers may not attend camp without all necessary completed health forms. Ample time is needed to review forms to ensure all documents have been completed in full and that no further information is needed. Be sure all fields have been completed by a parent/guardian and physician accordingly.

Please complete and return all camp forms as soon as possible.

How to Return Completed Forms

Parents have three options to return completed forms to Ambler Farm before the due date:

Email all forms as PDF’s only to our Medical Director at campforms@amblerfarm.org.

Postal Mail

Mail completed forms to the following address:

Drop Off

Drop off forms in our Drop Box at the farm in front of the white Carriage Barn. Please do not come to the farm to drop off forms during camp hours (8:30am – 1:30pm) as the main driveway will be closed and our staff will be busy overseeing campers.
